New Patient Paperwork
Your provider is offering you the ability to download and print their office paperwork through this website. Please print out all pages and bring them to your initial, or next, appointment.
Once your file has downloaded to your computer, in order for you to view it you will need to input a unique password that should have been given to you by your provider.
If you do not have the password, please click the email link or call the phone number listed next to your provider’s name.
Please DO NOT contact Accurate Medical Billing directly for a password as we do not have access to that information.
